Assessment responses may be personal or sensitive. Provide only information you are comfortable submitting through the website. Do not submit medical records, Social Security numbers, government identification, financial-account credentials, or other highly sensitive records through an unsecured form or text message. Website submissions do not create a healthcare relationship or guarantee protection under health-privacy laws that apply only to covered healthcare providers or plans.

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Text messaging privacy

If you choose to receive text messages, we use your telephone number, consent record, and messaging history to provide the informational communications described at the point of opt-in. Consent is optional and is not a condition of purchase or of submitting a non-SMS inquiry. Message frequency varies. Message and data rates may apply. You may reply STOP to opt out or HELP for assistance.

We retain opt-in and opt-out records as reasonably necessary to honor your choices and demonstrate compliance.
